In most cases "Que cosa??" is used to show disbelief or disagreement on a certain statement as in Say whaat...?!!" As in requesting clarification or confirmation (What was that?)... Or to reflect certain disapproval of something just learned as in No waaay...!! Or can very well mean a request to repeat something just said due to misunderstanding...

Yeah, that's probably the best translation. We are use to use "que cosa" when we don't understand something, maybe because the person said the phrase too low, or is just because the phrase is kinda hard to believe (extreme case)
